The following forms may be useful to vendors who wish to do business with TDA.

Texas Application for Payee Identification Number

• Vendors should complete the Application for Texas Identification Number form to obtain a Payee Identification Number, which is required to do business with the State of Texas.

Vendor Direct Deposit Authorization

• In order to receive payments by direct deposit, complete the Vendor Direct Deposit / Advance Payment Notification Authorization form.

Advance Payment Notification Authorization

• If you want to receive advance email or fax notifications when TDA makes a payment to you, complete the Advance Payment Notification Authorization Form.
